An improper placement, or one made with inadequate information, can end tragically for the dog and adoptive family. This comprehensive adoption application may seem formal, but is intended to ensure the right dog is being placed with you. All information provided will be kept confidential. Please fill out the adoption application below or return the downloadable application. Applications will be held for six months.

Adoption requirements are as follows:

  • You must be a Florida Resident 

  • You must not be gone from the home more than 4 hours per day

  • Children living in the home must be over the age of 10

  • A fenced yard or enclosed patio

  • Current pets are up to date on vaccines, are on heat worm preventive, and are spay/neutered

If you rent, do you have written permission from your landlord to have a dog over 50lbs? If yes, please include copy of letter.

Do you have a dog door leading to the outside?
If yes, will the dog have access when you are not home?
Have you ever owned a golden retriever?
Do you have any CURRENT pets in the home?
Does anyone in your household have allergies to dogs?
Who will have primary care of your golden?
Is anyone home regularly during the day?
Will your dog have access to the entire house?
Will you use a crate?
Do you tie or chain your dog at anytime?
Will your dog ever be kept in the garage, live outside in the yard, or outside kennel?
Are there children living in the home or visit regularly?
Have you ever sold, given away, or surrendered a pet to a shelter?
I understand that golden retrievers shed.
I agree to provide ongoing flea, tick, and heartworm preventative medicine or treatment.
I am aware that routine care and maintenance of a golden retriever averages between $800 and $1,000 per year and am prepared for these expenses.
I am aware that golden retrievers are large friendly dogs that may jump and could knock down children and elderly people.
I agree that I will provide additional training if needed for my golden.
Will your golden be allowed on the furniture? Some are accustomed to being allowed on furniture, some are not.
Would you prefer a male or female?
Would you consider adopting a dog with special needs such as physical or sensory limitations, or a dog that requires medication for a chronic controlled condition?
Would you consider a golden mix?
Would you use a groomer or groom your golden yourself?
We would like a reference from your current veterinarian, would you authorize us to contact them and allow them to release confidential informatin about you and your pets?

I/we understand that completing this application does not guarantee adoption of a dog from Joshua's House Golden Retriever Rescue. JHGRR reserves the right to refuse to adopt to any applicant. We also understand that JHGRR will work to provide us with a dog suitable to our personalities and lifestyles. We also agree to a home visit from volunteers of JHGRR to meet with us and our current family of pets.
